• Immediately After service ( see church clerk)
Documentation needed:
• Valid photo ID (Non-Expired Driver License or State I.D.) for correct address
• Proof of income (Award Benefit Letter, stub)
• Valid working phone number
*You are allowed to register for someone else if the above items are present along with a signed statement from that person with permission to do so.
Distribution Dates: TBA Time: Based on delivery driver
